Established Patients enjoy direct communication with Dr. Fishberg through the portal at the PORTAL and all function below are available on the portal as well.
New Patients register at PRE-REGISTRATION FORM and send/sms/text the front and back of your insurance card to 855-905-6346. We will verify your insurance and call you back to set up an appointment.
Appointments
We know you have many choices when choosing a family physician in Orlando, FL, so we have made requesting an appointment a simple process via our portal. If you cannot keep a scheduled appointment or will be delayed, please call us as soon as possible.
